This policy outlines your rights- and my obligations to you- in regards to your personal information. As a psychotherapist, I am required to collect and store personal information from my clients  and maintain a patient record for everyone I provide services for. As all healthcare professionals, I am bound by confidentiality, and I take the protection of your data with the utmost seriousness. This document specifies what information I collect for my patient records, how I store the gathered data, and  policies regarding the sharing of this data.

3.     Name of Record

 Joanna Kraus, LMHC patient record and psychotherapy notes.

 4.     Purpose of Record

 According to law, healthcare professionals are required to create and maintain a patient record. The information collected about you helps me formulate treatment plans, provide effective and personalized psychotherapy for you, and assess your progress in therapy.

 5.     Information Collected

  • Name

  • Date of Birth

  • Social security number/ personal identity code

  • Contact information

  • Dates of service

  • Treatment plan

  • Progress

  • Other information relevant to patient care and treatment

  • Notes of every visit

Only necessary and relevant information will be recorded. My policy is to keep my notes brief, general, and to the minimum required.

 6.     Source of Information

 I get this information mainly directly from you- from initial contact to each individual session.

 7.     Release of Information

 Information will only be released when required by law, or when requested by the patient via a completed and signed Release of Information form.

 8.     Transfer of Data Outside the EU

 Data will not be transferred outside the EU.

 9.     Data Storage

 Data is stored in files that are password-protected. These files are stored on a password-protected device, which remains in only my possession. The information is only accessible by me. Any paper forms are destroyed as soon as the information contained in them is input into the electronic file.

 10.  Right to Examine and Request Correction

 Each patient has the right to examine their own record and request that incorrect or incomplete information be corrected.

 11.  Length of Storage

 Records will be maintained for the length of time required by applicable law.
